So how do you know if these cars run or not? Is it just the luck of the draw thing, a gamble? The M5 caught my attention if there is a way to verify mechanical condition.

The icon next to the vehicle title refers to R=run and drive, S=engine starts and E=enhanced vehicle, which means nothing really. This is a dealer vehicle with DMV fees due, so it most likely has engine problems, otherwise it would have an "S" for engine starts.
